Basic FSSAI Registration
- Passport-size photograph of the applicant
- Proof of business address (rent agreement / utility bill)
- Food Safety Management System (FSMS) declaration
- Government-issued photo ID (Aadhaar / PAN / Voter ID)
- Business name and nature of food products
State FSSAI License
- Form B duly completed and signed by the proprietor/partner/director
- Proof of business premises (rental deed / ownership document)
- List of food products to be manufactured or processed
- Blueprint/layout plan of processing unit (if applicable)
- List of equipment and machinery used
- Authority letter with name and address of responsible person
- Photo ID and address proof of directors/partners/proprietor
Central FSSAI License
- Form B completed and signed
- Proof of business incorporation (COI / Partnership deed)
- List of directors/partners with contact details
- Analysis report of water to be used in food production
- Certificate of Ministry of Commerce for 100% EOU (if applicable)
- Import Export Code (IEC) certificate (for importers/exporters)
